How to File a Claim to your Insurance

To get started:
- Select your insurance provider.

- Attach Receipt and Prescription.
- Submit/Mail it to your insurance ( Claim Form, Receipt and Prescription)
- Wait 2-3 weeks to get a response from your insurance provider.
